Follistatin is a single-chain gonadal protein that specifically inhibits the release of follicle-stimulating hormone (FSH). The FST gene produces two isoforms, FST317 and FST344, containing 317 and 344 amino acids respectively, resulting from alternative splicing of precursor mRNA. Follistatin binds directly to activin, acting as an activin antagonist and specifically inhibiting the biosynthesis and secretion of pituitary FSH.

Follistatin is being investigated for its role in regulating muscle growth as an antagonist to myostatin (GDF-8), a member of the TGF-β superfamily that limits excessive muscle development. Research by Lee & McPherron demonstrated that inhibiting GDF-8—either through genetic knockout in mice or by increasing follistatin levels—leads to significantly increased muscle mass. Further studies in 2009 with macaque monkeys showed that modulating follistatin via gene therapy also promoted muscle growth and enhanced strength.
